How does this happen?

Broken loops can happen for a variety of reasons. Being one of the most important joints of the fly line, and one that gets the most use, it sees constant flexing and tension. The loop to loop connection with your leader rubs over time and many loops fail due to wear and simply break.

Another common broken loop issue is the line cracking right below the loop. This leads to an exposed core, which allows water to penetrate up the fly line, leading to sinking and long-term damage. The hinge at the loop also affect accuracy.
